Why Wisdom Teeth Cause Problems

Third molars arrive last, usually between 17 and 25, and most jaws no longer have room. A tooth that cannot fully erupt may come in at an angle, press against the neighboring molar, or remain partly covered by gum tissue where bacteria collect and no brush can reach.

That is how impacted wisdom teeth produce infections, decay in the second molar, cysts, and crowding pressure — frequently before any pain appears.

Plenty of wisdom teeth cause no symptoms and still need removal based on what the X-ray shows. The reverse is also true — fully erupted, cleanable wisdom teeth can stay.

The Procedure

We begin with imaging that maps each tooth's position and its relationship to the nerve and sinus. Straightforward extractions are done with local anesthesia; impacted teeth may require a small incision and sectioning of the tooth so it comes out through a smaller opening.

Nitrous oxide and oral sedation are available for patients who want them, and complex cases can be scheduled with IV sedation at our Bowling Green or South Warren offices within the same practice.

Recovery and Aftercare

Swelling generally peaks on the second or third day and then improves steadily. Ice the first day, moist heat afterward. Soft foods, plenty of fluids, and no straws, smoking, or vigorous rinsing for several days — that is what protects the clot and prevents a dry socket.

Most young patients are back to school or work within two to three days. You go home with written instructions and a way to reach our team if anything concerns you during healing.

L-PRF for Faster Healing

For surgical extractions we may recommend L-PRF, which concentrates healing factors from a small sample of your own blood and places them directly in the extraction site. Patients commonly report less swelling and a smoother recovery.

When Wisdom Teeth Can Stay

Removal is not automatic. Some patients have jaws with genuine room, and a wisdom tooth that erupts fully upright, meets its opposing tooth properly, and can be brushed and flossed like any other molar has earned its place. In those cases we monitor it with periodic X-rays and leave it alone.

What we watch for over time is decay on the back surface, gum inflammation that keeps recurring, and bone loss between the second and third molars. If those appear, removal becomes the conservative choice — protecting the second molar, which is a far more valuable tooth.
